Liability & Release Waiver
Please read the following terms carefully. Every parent or legal guardian must acknowledge and sign this waiver before an athlete may participate in any Crowned Homeschool Training training session.
1. Assumption of Risk
I understand that participation in athletic conditioning, sport-specific training, and group activities involves inherent risks, including but not limited to bumps, bruises, sprains, fractures, and other physical injuries. I voluntarily accept and assume all such risks on behalf of my child.
2. Release of Liability
In consideration of my child's enrollment, I hereby release, waive, and discharge Crowned Homeschool Training, its coaches, staff, volunteers, and affiliated facilities from any and all liability, claims, demands, or causes of action arising from ordinary negligence during participation.
3. Medical Emergency Consent
In the event of injury or illness during a session, I authorize Crowned Homeschool Training staff to administer reasonable first aid and, if necessary, secure emergency medical treatment for my child. I agree to be financially responsible for any medical services rendered.
4. Participation Rules & Conduct
My child agrees to follow all coach instructions, respect teammates and equipment, arrive on time, and wear appropriate athletic gear. Crowned Homeschool Training reserves the right to dismiss participants whose conduct endangers themselves or others, with no refund.
5. Media & Photo Release
I grant Crowned Homeschool Training permission to use non-identifying photographs and video of my child captured during training for promotional purposes. I may revoke this permission in writing at any time.
How to sign this waiver
The waiver acknowledgment checkbox and digital signature field are presented as the final step of your registration. Once your payment is approved (or your charter pathway is submitted), a compiled copy of this waiver is automatically emailed to the parent on file and stored in your Parent Portal document vault.
